Hourglass Syndrome and Its Effect on the Body

August 3, 2026 By Megan Thompson

If you’ve spent any time on TikTok or Instagram lately, you may have seen people talking about “hourglass syndrome,” a term for a habit most of us picked up without even realizing it: sucking in the stomach and holding it there, all day, every day.

It is not a formal medical diagnosis, and it sounds harmless. But if you’re doing this behavior constantly, it can quietly contribute to back pain, shallow breathing, digestive discomfort, and pelvic floor issues.

Many of us grew up with magazines and social media pushing a particular beauty standard, which led some of us to develop this chronic habit. It may have started as a conscious choice, but over time, it became something we did without thinking — to the point where, for some of us, relaxing the stomach muscles now feels harder than gripping it. Does this sound familiar?

Read on to learn more about this habit, how it can affect your body, and how we can help.

What Is Stomach Gripping?

Stomach gripping is the habit of tightening your upper abdominal muscles to pull your stomach up and in, usually to look leaner or flatter. Over time, this can change the way your body actually moves and breathes.

Your diaphragm, the dome-shaped muscle beneath your lungs, is supposed to move down and outward when you inhale, letting your ribs and belly expand. When you’re chronically gripping, your diaphragm and upper abs work against that motion instead of with it.  Your lower ribs get pulled inward, your upper abs become overworked and tight, and your lower core muscles– the ones actually meant to stabilize your spine and pelvis– go underused and weaken.

The name “hourglass syndrome” comes from the pinched, narrow-waisted shape this can create at the ribcage, sometimes along with a crease or dimpling near the belly button.

Why It Matters

A little core engagement here and there isn’t a problem. The issue is when gripping becomes your default, all-day posture.  Left unaddressed, it can contribute to:

  • Shallow, upper-chest breathing instead of full, diaphragmatic breaths
  • Neck and shoulder tension as your chest and shoulder muscles get pulled into compensating for the breath your diaphragm should be taking, leaving them chronically overworked
  • Low back as other muscles compensate for stabilizers that aren’t doing their job
  • Poor pelvic floor function since the diaphragm and pelvic floor work as a team
  • Digestive discomfort from constant tension in the upper abdomen
  • Rib cage and thoracic spine stiffness from the ribs being held in an unnatural position for hours at a time
  • Neck and shoulder tension as your chest and shoulder muscles get pulled into compensating for the breath your diaphragm should be taking, leaving them chronically overworked
When the diaphragm can’t move freely, breathing shifts up into the chest and shoulders

Signs You Might Be a “Stomach Gripper”

  • Your stomach feels tense even when you’re just sitting or standing still
  • You feel like you can’t take a full, deep breath without effort, or your breath feels like it stops in your upper chest
  • You notice a crease or dimpling around your belly button
  • You have chronic low back tightness and/or shoulder pain that doesn’t fully resolve with stretching
  • You’ve been told you have “tight hips” or “weak glutes” despite regular exercise

How Chiropractic Care and Massage Therapy Can Help

Chiropractic adjustments help restore mobility to the thoracic spine and rib cage, which often become restricted from years of holding this braced position. When your ribs and mid-back move more freely, your diaphragm has the room it needs to actually do its job.

Our chiropractors can recommend breathing exercises to retrain the diaphragm’s natural up-and-down movement, helping you break the shallow, upper-chest breathing pattern that gripping creates.

Therapeutic massage, on the other hand, releases the chronic tension that builds up in the upper abdominals, chest, and surrounding muscles from constant gripping, making it easier for your body to relearn a more natural breathing and bracing pattern.

Together, these approaches address both the joint restriction and the soft tissue tension that keep this cycle going, which stretching or willpower alone usually can’t fix.

The First Step Is Awareness

Changing a long-standing habit takes time, especially if it’s been your default for years. But it starts with something simple: noticing when you’re gripping. That awareness on its own is often where the real change begins.

If you’ve been dealing with unexplained back pain, tightness, or a feeling that you can never quite take a full breath, it may be worth having it looked at.  We can assess your posture, breathing mechanics, and spinal and rib mobility, and put together a plan to help your body move the way it’s meant to.

School is back, routines are back, and so is a lot more time hunched over computers and homework. We’ve been seeing plenty of tight necks and shoulders lately.
